Hauling Sacks

Help the Blacksmith

Then, engage with the nearby woman, Fanka, who works as a cook in the noble's kitchen. Offering to help her grants you access to the kitchen. Follow Fanka to the kitchen; while you don't need to lockpick it for her, doing so boosts your reputation and allows you to obtain food whenever needed.
Speak with the Chamberlain

Make Fever Tonicum

Head to the stairs opposite the forge and enter the kitchen used by non-nobles. If sneaking, give the guard the food to make him leave, then head upstairs. Navigate through the halls until you reach the surgeon's workshop on the right. Craft the Fever Tonicum and, if you convinced the chamberlain, also make a Digestive Potion using the ingredients in the room's chest.
Cure Thomas

Return to the noble's kitchen and ascend the stairs to avoid additional guards. Stay crouched and move carefully. When you encounter a guard at the bottom of a staircase leading to Thomas' room, throw a pebble to distract him and sneak past. Continue up the stairs to find Thomas in bed with his sister, Adele. Give Adele the Fever Tonicum, triggering the quest's concluding cutscene.
